BAYADA Home Health Care in Burlington County, NJ, is seeking a full-time Physical Therapy Assistant (PTA) to join our Burlington East Visits team. You will provide one-on-one home health therapy visits to adult and geriatric clients in their homes, collaborating with a licensed Physical Therapist.

Qualifications

• Current New Jersey Physical Therapist Assistant (PTA) license.
• Graduate of a Physical Therapist Assistant program approved by the American Physical Therapy Association (APTA), OR meet applicable experience and proficiency requirements.
• Ability to work independently and effectively manage time.
• Strong communication, interpersonal, and organizational skills. Home health experience preferred, but not required.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel to client homes throughout Burlington County.

Responsibilities

• Provide one-on-one physical therapy visits to clients in their homes throughout Burlington County.
• Partner with an experienced and supportive Physical Therapist to provide individualized care.
• Provide skilled physical therapy treatments and interventions as delegated by a licensed Physical Therapist.
• Implement individualized plans of care, including therapeutic exercise, gait training, balance training, mobility, and other interventions.
• Help clients improve or maintain strength, mobility, safety, and functional independence.
• Monitor client progress and communicate changes or concerns to the supervising Physical Therapist.
• Educate clients, family members, and caregivers on therapeutic exercises, mobility techniques, safety strategies, and rehabilitative care.
• Collaborate with members of the interdisciplinary care team to support positive client outcomes.
• Complete timely and accurate documentation using BAYADA's electronic medical record (EMR) system.
