Job summary

SunnyBrook Home Care seeks a Full-time Physical Therapy Assistant (PTA) to join our home health team. You will deliver skilled PT services in patients’ homes, educate families, and maintain accurate same-day documentation. A current Iowa PTA license and valid driver’s license are required. Mileage reimbursement and flexible scheduling are offered.

Qualifications

• PTA license in Iowa is required.
• Minimum of 1 year as PTA in Home Care or acute care.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Plan of Care: provide skilled physical therapy as ordered and monitor patient conditions.
• Minimize Patient Risk: assess functional problems and work with PT to improve status.
• Family Educator/Advocate: educate patient and family on disease/injury and use of equipment.
• Policies: document according to agency protocol and Medicare guidelines.
• Rules and Regulations: follow policies and communicate schedule changes.
