Qualifications

• Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment.

Responsibilities

• Provide physical therapy services to patients according to physician's plan of care.
• Assess and evaluate therapeutic, rehabilitative/functional status.
• Evaluate the patient’s home environment and assess for muscle strength.
• Direct patients in the use and care of therapeutic appliances.
• Identify patient needs for other home health services and refer as necessary.
