Job summary

Fox Rehabilitation in Myrtle Beach, SC seeks licensed Physical Therapists to deliver one-on-one care in homes and senior living communities. Treat patients to improve mobility and independence using the Geriatric House Calls model, with flexibility to schedule patients and include telehealth when suitable.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and functional independence.
• Document effectively within FOX's EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
