Job summary

Qualifications

• Licensed physical therapist in the state of employment.
• Minimum two years of experience.
• Automobile liability insurance and current unrestricted driver’s license.
• CPR certification is required.

Responsibilities

• Evaluates and treats patients in home settings to restore function and reduce pain.
• Travels to different home care settings and clinics as needed.
• Assess home environments for rehab and independence.
• Maintain accurate care progress records per policy.
• Supervise assistants and contribute to staff education when needed.
• Educate patients and caregivers on PT techniques and equipment.
