Job summary

Seaport Scripps Home Health in Oceanside, CA is seeking a licensed Physical Therapist to provide in-home therapy across the South Coastal Territory. The PT will deliver care per a physician’s plan, helping patients regain mobility and independence.

This full-time role offers a $5,000 sign-on bonus, competitive pay of $55–$70 per hour, and comprehensive benefits including health, dental, vision, life and disability insurance, plus a 401(k) plan with company match.

Qualifications

• Active California Physical Therapy License.
• Minimum 2 years PT experience, home health preferred.
• Strong communication and organizational skills.
• Current CPR certification.
• Valid driver’s license and insured vehicle.

Responsibilities

• Evaluate patients’ functional status and develop care plans.
• Assess home environments and recommend modifications.
• Provide direct physical therapy treatments.
• Educate patients and caregivers on techniques and equipment.
• Monitor progress and adjust treatment plans.
• Collaborate with physicians and interdisciplinary teams.
• Document clinical notes and progress reports.
• Participate in discharge planning.
• Supervise Physical Therapy Assistants per policy and regulations.
