Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence in their homes, reporting to the Clinical Manager. You will deliver personalized care focused on functional improvement to enhance quality of life.

The role involves planning and administering PT in patients' homes, assessing strengths, developing rehab programs, and supervising PTAs/Home Health Aides. Schedule 40 hours/week with a pay range of $93,000–$128,000 per year.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to improve function.
• Assess and document patients' strengths to guide treatment goals.
• Develop rehab programs aligned with physician Plan of Treatment.
• Instruct and supervise PTAs and Home Health Aides with clear care plans.
• Document observations, interventions, and progress promptly.
• Report progress to physician, Clinical Manager, care team, patient and family.
