Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one care in home and senior living settings in Selinsgrove, PA. You’ll help older adults improve mobility, independence, and quality of life through the Geriatric House Calls model. Flexibility to schedule visits and incorporate virtual care is available.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, independent living, assisted living, memory care).
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
