Fox Rehabilitation in Selinsgrove, PA is seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes or within senior living communities. The Geriatric House Calls™ model emphasizes autonomy, flexible scheduling, and meaningful patient outcomes.

Responsibilities include evaluations, developing plans of care, delivering evidence-based therapy, coordinating with an interdisciplinary team, and documenting in FOX's EMR.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilités

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support mobility, independence, and function.
• Document effectively within FOX's EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.
