Job summary

Cleveland Clinic in Independence Township, OH, is seeking a dedicated Physical Therapist to deliver home-based care, helping patients increase strength, mobility, endurance and coordination in familiar surroundings.

You’ll work 8:00 AM–5:00 PM with weekend and holiday rotations; collaborate with a care team, build strong patient relationships, and use mobile documentation and technologies to provide high-quality, patient-centered care.

Qualifications

• Bachelor’s degree or certificate.
• Graduate of a Physical Therapy Program accredited by the APTA.
• Current state licensure as a Physical Therapist (PT).
• Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross.
• Valid Driver’s License (VDL) and proof of $100,000/$300,000 automobile insurance coverage.

Responsibilities

• Evaluate and conduct medically prescribed physical therapy treatment programs.
• Perform initial and ongoing assessments.
• Establish, revise and evaluate plans of care.
• Perform physical therapy interventions utilizing standard physical therapy techniques and skills appropriate for the patient’s age and condition.
• Supervise the activities of physical therapy assistants, students and other support caregivers.
• Maintain complete and up-to-date documentation and files at all times regarding patient condition and the effectiveness and timeliness of care and interventions.
• Participate in ongoing interdisciplinary assessments of assigned patients.

Education

Bachelor’s Degree or certificate

Graduate of a Physical Therapy Program accredited by the American Physical Therapy Association
