PARS Therapy in Jefferson, TX is seeking a compassionate Physical Therapist (PT) to join our home health team. You will evaluate patients at home, develop personalized treatment plans, and apply evidence-based interventions to improve mobility, reduce pain, and enhance independence.

We value patient-centered care, strong communication, and collaboration with an interdisciplinary team. Texas licensure and liability coverage are required; prior home health experience is preferred as we support

Qualifications

• Active TX Physical Therapist license.
• Professional liability insurance coverage.
• Graduate degree or higher in Physical Therapy.
• Experience in home health or rehabilitative settings preferred.
• Strong ability to adapt care approaches to patient needs.
• Up-to-date knowledge of evidence-based treatment techniques.
• Excellent communication and professional demeanor.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function, mobility, and alleviate pain.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust plans to achieve optimal outcomes.
• Document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and occupational therapists.
• Conduct home safety assessments and recommend fall prevention or re-injury strategies.
• Deliver compassionate care while empowering patients to achieve greater independence.
