Job summary

CenterWell Home Health in Fort Mill, SC seeks a Home Health Physical Therapist to help patients regain strength, mobility and independence in their homes, reporting to the Clinical Manager.

You will work with a patient-centered care team, perform comprehensive evaluations, and develop rehab plans aligned with physician orders. LP licensure and CPR are required, with a focus on teamwork and exceptional patient care.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• One year of experience as a Physical Therapist.

Responsibilities

• Plan and administer prescribed PT in patients' homes to improve physical function.
• Assess and document patients' strength to guide physician evaluation and goals.
• Develop rehab plans aligned with physician orders and patient goals.
• Provide instructions to PTAs and HHAs and supervise documentation.
• Document observations and evaluations promptly and thoroughly.
• Report progress to physician, Clinical Manager, care team, patient and family.
