Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength and independence in their homes, reporting to the Clinical Manager. You will plan and administer prescribed physical therapy and document progress to support physician evaluations and goals.

Responsibilities include assessing patients, developing rehabilitation plans, guiding PTAs and Home Health Aides, and communicating progress to care teams and families.

Qualifications

• Current and unrestricted Physical Therapy license required.
• Current CPR certification required.
• Experience collaborating with a healthcare team is required.

Responsibilities

• Plan and administer prescribed PT in patients' homes to restore function.
• Assess patients' physical strengths to guide treatment goals.
• Develop a rehabilitation program aligned with the physician’s plan of treatment.
• Provide clear instructions to PTAs and Home Health Aides and oversee documentation.
• Document patient observations, interventions, and progress.
• Report progress to physician, Clinical Manager, care team, patient and family; submit evaluations and discharge summaries.
