UnitedHealth Group is seeking a Physical Therapist to provide skilled services in patients’ homes, promoting mobility, restoring function, and improving safety. You will conduct in‑home evaluations and develop individualized treatment plans in alignment with physician orders and patient goals.

The role involves collaborating with patients, caregivers and an interdisciplinary team to support recovery and prevent hospitalizations, while delivering evidence‑based care in the home setting.

Qualifications

• Current license to practice physical therapy in the state you're assigned to work in.
• Graduate from a CAPTE-accredited physical therapy education program.
• Current CPR certification.
• 1+ years related physical therapy experience for PTs entering home health care to ensure knowledge and skills to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Driver’s License and access to reliable transportation.

Responsabilités

• Help patients maintain, restore and improve movement, activity and function.
• Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning.
• Collaborate with patients, caregivers, and an interdisciplinary care team to support recovery, prevent hospitalizations, and improve quality of life within the home setting.
