Job summary

Choice Health at Home is seeking a compassionate Physical Therapist to assess, plan, and provide rehabilitative care in patients’ homes in Broken Arrow, OK. You will help patients regain mobility and function while developing individualized treatment plans and ensuring high-quality care.

The role requires an accredited PT program completion, current state PT license, CPR certification, and a valid driver’s license, with home health experience preferred.

Qualifications

• Successful completion of an accredited physical therapy program.
• Current and valid state Physical Therapist license.
• Current CPR certification.
• Valid driver’s license and automobile insurance.
• At least one year of home health experience preferred.
• Experience with Homecare Homebase preferred.

Responsibilities

• Perform and document comprehensive initial evaluations and establish a diagnosis and plan of care.
• Develop and implement individualized treatment programs to improve function and relieve pain.
• Evaluate patient responses and modify interventions as needed.
• Apply infection-control practices and universal precautions.
• Ensure accurate, timely documentation of care.
• Communicate effectively with patients, families, and the healthcare team.
• Educate patients and caregivers on treatment plans and safety practices.
• Uphold the agency’s standards and policies.
• Complete all required documentation within company guidelines.
