Job summary

Choice Health at Home in Broken Arrow, OK is seeking a compassionate Physical Therapist to assess, plan, organize, and provide rehabilitative care in patients' homes. You will help patients improve mobility, relieve pain, increase strength, restore function, and manage conditions resulting from illness or injury.

The role requires completing initial evaluations, designing individualized treatment plans, documenting progress, and communicating with patients and the care team.

Qualifications

• Successful completion of an accredited physical therapy program.
• Current and valid state Physical Therapist license.
• Current CPR certification.
• Valid driver’s license and automobile insurance.
• At least one year of home health experience preferred.

Responsibilities

• Perform and document initial evaluations, analyze findings, and establish diagnosis and plan of care.
• Develop and implement individualized treatment programs to improve function and alleviate pain.
• Evaluate patient progress and modify interventions as needed for optimal benefit.
• Apply infection-control practices and universal precautions during care.
• Ensure accurate documentation and continuity of care.
• Communicate with patients, families, caregivers, and the interdisciplinary team.
• Educate patients on treatment plans, exercises, safety, and goals.
