UPLIFT HOMECARE, LLC is seeking a compassionate Licensed Physical Therapist (PT) or Physical Therapist Assistant (PTA) for a part-time home health role serving Ft Meade and surrounding areas. You will provide one-on-one patient care in patients' homes and collaborate with a supportive clinical team.

Qualifications

• Must hold an active Florida PT or PTA license.
• Graduate of an accredited physical therapy program.
• Must have passed the national licensure examination.
• Minimum of 1 year of home health experience preferred; strong documentation skills.

Responsibilities

• Provide therapeutic exercises, treatments, and interventions to relieve pain and restore function.
• Observe, document, and report patient responses and condition changes.
• Assist in evaluating patient functional abilities and revising care plans as needed.
• Complete clinical notes and progress reports accurately and within required timeframes.
• Participate in care coordination conferences and interdisciplinary communication.
• Educate patients and caregivers on exercises, mobility, safety, and treatment goals.
• Participate in quality improvement initiatives and in-service education programs.
• Maintain current knowledge of physical therapy practices and standards.
