Job summary

Magnolia Home Health in Austin, TX seeks a PRN Physical Therapist to provide patient-centered therapy in home settings. You will evaluate patients, develop plans of care, and deliver therapy with limited supervision, coordinating with families and care teams.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Two years of physical therapy experience within last five years.
• Current physical therapy license and CPR certification.
• Valid driver’s license and auto liability insurance.

Responsibilities

• Conducts therapy within defined standards of care per APTA guidelines and plan of care.
• Designs and implements a Plan of Care for the patient and significant others.
• Provides direct patient care and reassesses patient needs for optimal outcomes.
• Instructs patients and families in home exercise programs and recommends assistive equipment as needed.
• Completes and submits documentation timely and accurately per policy.
