Orion Placement in California is seeking a licensed Physical Therapist for in-home care in Redlands. The role is hybrid, traveling to patients' homes throughout the Redlands territory, with a pay range of $48.00–$71.00 per hour and a $10,000 hiring bonus.

You will assess mobility and function, develop personalized treatment plans, educate patients and caregivers, and collaborate with a multidisciplinary team to deliver high-quality in-home therapy.

Qualifications

• Active California Physical Therapist (PT) license required.
• Home health PT experience preferred.
• 1+ year of Physical Therapist experience preferred.
• Master's or Doctorate degree in Physical Therapy preferred.
• BLS/CPR certification preferred.
• Valid driver's license and reliable insured vehicle preferred.
• HCHB/Homecare Homebase experience preferred.
• OASIS experience preferred.

Responsabilités

• Evaluate patients in their homes and assess mobility, strength, coordination, pain, and functional limitations.
• Develop individualized physical therapy treatment plans based on each patient's needs and goals.
• Provide therapeutic exercises and interventions designed to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on exercises, safety precautions, and strategies for continued progress.
• Monitor patient outcomes and adjust treatment plans as appropriate.
• Collaborate with nurses, physicians, therapists, and other members of the interdisciplinary care team.
• Maintain accurate and timely documentation of patient progress and treatment plans.
• Travel throughout the assigned Redlands territory to provide high-quality in-home care.

Location:Redlands, CA - Hybrid. This is a field-based home health role where you will provide care in patients' homes throughout the Redlands territory while completing documentation from the field.

Note:Candidates must have an active California Physical Therapist (PT) license. Home health experience, 1+ year of Physical Therapist experience, a Master's or Doctorate degree in Physical Therapy, BLS/CPR certification, a valid driver's license, reliable transportation, HCHB/Homecare Homebase experience, and OASIS experience are preferred.
