Job summary

Fox Rehabilitation in Clemson, SC is seeking a licensed Physical Therapist to deliver one-on-one geriatric care in homes and senior living communities. You’ll conduct evaluations, create individualized plans of care, and provide evidence-based therapy while building lasting patient relationships with flexible scheduling and the option for telehealth where appropriate.

You’ll join a clinician-driven team that supports autonomy, ongoing CE, and advancement opportunities as part of the Geriatric

Qualifications

• Active Physical Therapy license in the state of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Current CPR certification and basic computer/EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support mobility, independence, and wellness.
• Document in FOX’s EMR in compliance with regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
