Job summary

FOX Rehabilitation in New York is seeking a licensed Physical Therapist to deliver in-home and senior living care. You will evaluate patients, develop individualized plans, and provide evidence-based therapy in private residences and communities.

Join a clinician-operated practice that supports telehealth options, flexible scheduling, and interdisciplinary collaboration with PTs, OTs, SLPs, and Fitness Specialists. Base pay in NY ranges with potential for more based on experience and roles.

Qualifications

• Valid Physical Therapy license in state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they live.
• Perform evaluations, develop individualized plans, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility and independence.
• Document in FOX’s EMR system per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth / virtual care when appropriate

Education
