Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care in homes and senior living communities in Allentown, NJ, with flexibility to include telehealth as part of the schedule. You will evaluate patients, create individualized plans, and provide evidence-based therapy aimed at improving mobility and independence.

In-home visits and remote options support a broad patient base. The role offers autonomy, scheduling control, and opportunities for professional growth

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in their homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
