Job summary

INTEGRIS Health invites a Home Health Physical Therapist to join our team in Oklahoma City, OK on a day shift, full-time basis. The role focuses on in-home evaluations, treatment planning, and ensuring high-quality patient outcomes within our home health network.

The ideal candidate has 2 years of acute care or rehabilitation experience, graduated from an accredited PT program, and holds an Oklahoma driver’s license with a good driving record. Excellent English communication is required.

Qualifications

• 2 years experience in an acute care or rehabilitation setting.
• Graduated from an accredited Physical Therapy program.
• Must communicate effectively in English (verbal/written).
• Oklahoma Driver's License with acceptable driving record.

Responsibilities

• Interprets referrals, evaluates plan of care and expected outcomes for patients.
• Performs comprehensive physical and safety assessments.
• Plans and provides home health physical therapy treatment with periodic re-evaluation.
• Coordinates patient care with clinical team and directs assistants/service personnel.
• Maintains documentation in accordance with home health policies.
