Job summary

Fox Rehabilitation in West Seattle, WA is seeking licensed Physical Therapists to deliver one-on-one geriatric care in home and senior living settings. You’ll evaluate, plan, and provide evidence-based therapy, with flexibility to include telehealth and virtual care as part of your schedule.

Qualifications

• Must have a valid Physical Therapy license in the practicing state or be eligible to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency; CPR certification required.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or community settings.
• Perform evaluations, develop care plans, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility and independence.
• Document in FOX's EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
