Job summary

FOX Rehabilitation is hiring licensed Physical Therapists to provide one-on-one in-home and senior living community care in Bremerton, WA. You will evaluate patients, create individualized care plans, and deliver evidence-based therapy to improve mobility and independence.

Work includes in-home visits, telehealth options, and collaboration with PTs, OTs, SLPs, and Fitness Specialists in a supportive, clinician-driven culture. Competitive benefits and career growth available.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy in patients' homes or community settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver sessions that support wellness, longevity, mobility, and functional independence.
• Document effectively in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
