Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to join our Bellevue, WA team delivering one-on-one geriatric care in patients' homes or Senior Living Communities. You will conduct evaluations, create individualized plans, and provide evidence-based therapy to improve mobility and independence.

The role offers autonomy in scheduling, opportunities for telehealth, and a collaborative, interdisciplinary team, with full-time, part-time, and PRN pathways available.

Qualifications

• Must hold a valid Physical Therapy license in the state(s) of practice or be eligible to obtain.
• Degree from an accredited Physical Therapy program (DPT).
• Basic computer and EMR proficiency.
• Current CPR certification is required.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
