Job summary

FOX Rehabilitation in Rochester, MN is seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes or senior living communities. This role emphasizes autonomy, scheduling flexibility, and meaningful patient outcomes within our Geriatric House Calls model.

The position supports treating across private residences and senior living settings, with telehealth options to extend access and continuity of care.

Qualifications

• Active Physical Therapy license in Minnesota.
• Degree from an accredited Physical Therapy program.
• Current CPR certification and basic computer/EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in private homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver sessions to support mobility and functional independence.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate.
