Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to provide one-on-one, in-home care for older adults in Park Slope, NY. You will evaluate, plan, and deliver evidence-based treatment in private residences or Senior Living Communities, with the flexibility to manage your schedule and include telehealth where appropriate.

This role emphasizes autonomy, meaningful patient relationships, and collaboration with an interdisciplinary team.

Qualifications

• Must hold valid Physical Therapy license in the state of practice (NY).
• Graduate from an accredited Physical Therapy program.
• Basic computer and EMR proficiency; CPR certification current.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence and function.
• Document in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging in place.
• Offer telehealth/virtual care when appropriate.
