Job summary

Wealth Recruitment, LLC is seeking a compassionate Physical Therapist to deliver one-on-one home health services in Greenwich, CT. You will support patients to regain mobility, independence, and confidence, while enjoying autonomy and a flexible schedule.

Responsibilities include evaluating mobility, developing personalized plans, educating families, coordinating with a clinical team, and documenting progress. Requires CT license and 1 year of PT experience; home health experience preferred.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Connecticut Physical Therapist license.
• At least 1 year of experience as a practicing Physical Therapist.
• Home Health Physical Therapy experience preferred.

Responsibilities

• Provide high-quality, patient-centered physical therapy services in patients’ homes.
• Evaluate patients’ mobility, strength, functional abilities, and rehabilitation needs.
• Develop and implement individualized treatment plans focused on improving independence and quality of life.
• Monitor patient progress and adjust treatment plans to achieve the best possible outcomes.
• Educate patients and families on exercises, therapy techniques, safety, and lifestyle modifications.
• Collaborate with nurses, physicians, therapists, and other healthcare professionals to provide coordinated care.
• Maintain accurate and timely documentation of patient visits and progress.
