Job summary

InHome Therapy is hiring a licensed home care Physical Therapist (PT) to serve adult and geriatric patients in their homes in and around Oak Park. If you are looking for a flexible schedule, consistent caseload, and a team that supports you, we would love to hear from you.

The role offers mileage reimbursement, paid training, mentorship, and a per-visit compensation ranging from $65 to $85. A tablet with data plan is provided to support documentation and care delivery.

Qualifications

• Active physical therapy license in good standing.
• BLS CPR certification.
• Reliable transportation to see patients in their homes.
• Basic comfort with technology - email, texting, EMR systems (training provided)

Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients in your community.
• Evaluate patient mobility, strength, and function to develop individualized treatment goals.
• Implement effective rehabilitation and therapeutic exercise programs.
• Educate patients and families on recovery plans and preventive care.
• Maintain accurate and timely home health care documentation per Medicare and agency standards
