THRIVE NOW THERAPY LLC in Shelby, NC is seeking a passionate Home Physical Therapist to provide one-on-one care in patients’ homes. You will evaluate, plan, and implement individualized treatments to restore function and mobility, emphasizing safety and independence.

Formación

• Valid Physical Therapist (PT) license in North Carolina.
• Graduate of an accredited Physical Therapy program (DPT or MPT).
• Home health or outpatient physical therapy experience preferred.
• Strong knowledge of home health guidelines and documentation requirements.
• Excellent communication, time management, and organizational skills.
• Reliable transportation and valid driver's license required.
• Compassionate, self-motivated, and able to work independently in a home setting.

Responsabilidades

• Conduct in-home physical therapy evaluations and assessments.
• Develop and implement individualized treatment plans.
• Provide therapeutic interventions including therapeutic exercise, manual therapy, and gait training.
• Monitor and document patient progress, adjusting care plans.
• Educate patients and caregivers on home exercise programs and injury prevention.
• Collaborate with physicians, case managers, and interdisciplinary care teams.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.
