Job summary

Metro Physical & Aquatic Therapy is seeking a motivated Physical Therapist to provide in-home care across New York. All levels of experience are welcome; training is provided for new graduates.

A licensed NY PT or eligibility is required, with the ability to travel to patients’ homes across the designated geographic area. The position offers a competitive base salary, 1099 and W-2 options, full benefits, generous PTO, and opportunities for professional growth in a supportive team.

Qualifications

• Licensed Physical Therapist in NY or eligible.
• Home health experience preferred.
• Must be able to travel to patients' homes.

Responsibilities

• Accept and treat patients within designated geographic area.
• Evaluate home-based physical therapy needs and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
