Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver in-home and senior-living based physical therapy in Boca Raton, FL. The role emphasizes autonomy, scheduling flexibility, and meaningful patient outcomes, with opportunities for telehealth integration.

You will treat older adults, perform evaluations, and develop plans of care within a collaborative, interdisciplinary team, promoting independence and wellness in place.

Qualifications

• Valid Physical Therapy license in the state of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.

Responsibilities

• Provide one-on-one PT to older adults in-home or in senior living settings.
• Perform evaluations and develop individualized plans of care.
• Deliver evidence-based therapy to improve mobility and independence.
• Document care in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote aging in place.
• Provide telehealth / virtual care when appropriate.
