Job summary

Ascend Health Idaho is seeking a compassionate Physical Therapist to join our Home Health team in Idaho Falls on a PRN, contract basis. The PT will evaluate patients in their homes, develop individualized plans, and help improve mobility and independence.

The role requires licensure in Idaho, strong clinical skills, and the ability to work independently with flexible scheduling. Home health experience is preferred. Competitive PRN pay and support from a patient-centered team are offered.

Qualifications

• Current Idaho Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Home Health experience preferred (but not required).
• Strong clinical, communication, and time management skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation and valid driver\'s license.

Responsibilities

• Evaluate patients in the home setting and determine PT needs.
• Develop and implement individualized treatment plans.
• Provide skilled PT services per physician orders.
• Educate patients and caregivers on exercises, safety, and care plans.
• Document visits and patient progress in the EMR.
• Communicate with physicians and care team members to coordinate care.
• Maintain compliance with home health regulations and company policies.
