Job summary

Ascend Home Health is seeking a compassionate Physical Therapist to join our Home Health team in Idaho Falls on a PRN, contract basis. You will evaluate patients in their homes, develop personalized plans, and deliver skilled PT services to improve mobility and independence.

The role requires Idaho PT licensure, a PT degree, and strong clinical and communication skills. Home health experience is preferred, with reliable transportation and a flexible schedule supporting patient-centered care.

Qualifications

• Current Idaho Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Home Health experience preferred.
• Strong clinical, communication, and time management skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation and valid driver's license.

Responsibilities

• Evaluate patients' home-based physical therapy needs.
• Develop and implement individualized treatment plans.
• Provide skilled PT services according to physician orders.
• Educate patients and caregivers on exercises, safety, and care plans.
• Accurately document visits and patient progress in the EMR.
• Communicate effectively with physicians, care team members, and caregivers.
• Maintain compliance with home health regulations and company policies.
