Ideal candidates are licensed or eligible in MA, with strong communication skills and the ability to manage diverse caseloads while engaging families in care plans and home exercise programs.

Qualifications

• Stellar communication skills and patient-centered approach.
• Capable of managing a varied caseload.
• Licensed or eligible to practice Physical Therapy in Massachusetts.

Responsibilities

• Provide one-on-one therapy services in patients' homes.
• Assess patient needs, develop individualized treatment plans, conduct sessions, and monitor progress.
• Offer autonomous decision-making in care planning and treatment implementation.
• Conduct thorough in-home evaluations.
• Implement personalized care plans and educate on home exercise programs.
• Collaborate with healthcare professionals to ensure holistic care and maintain regulatory documentation.
• Ensure all physician orders are current.
