Qualifications

• Current MA Physical Therapist licensure or eligibility.
• Graduate from an accredited physical therapy program.
• Excellent communication and patient-centered approach.
• Ability to manage a varied caseload.

Responsibilities

• Provide one-on-one in-home therapy to patients.
• Assess patient needs and design individualized treatment plans; track progress.
• Autonomy in patient care decisions and treatment planning.
• Conduct thorough in-home evaluations.
• Educate patients and families about home exercise programs.
• Collaborate with healthcare team to ensure holistic care.
• Maintain documentation to meet regulatory requirements.
• Obtain and update physician orders as required.
