Job summary

Well Care Home Health of the Triangle is seeking a licensed Physical Therapist to evaluate, treat, and discharge patients in a home health setting across North Carolina. The role requires graduation from an accredited Physical Therapy program and state licensure (NC/SC) with CPR certification.

A minimum of 1 year of experience is preferred, along with strong documentation and communication skills. The physical demands include lifting >50 lbs, transferring >100 lbs, and frequent mobility tasks.

Qualifications

• Education: Graduate of Physical Therapy program from an accredited university.
• Licensure / Certifications: Licensure in the state in which providing therapy (NC/SC); CPR certification.
• Experience: Minimum 1 year.
• Essential Technical / Motor Skills: Knowledge of home health and physical therapy procedures; ability to use equipment and assistive devices; proper transfer/guarding of patients.
• Interpersonal Skills: Professional and positive interaction with the public, coworkers, patients, families, and physicians.
• Essential Physical Requirements: Lifting >50lbs, transferring >100lbs, bending, stooping, and other physical demands.
• Essential Mental Abilities: Ability to develop treatment programs, prioritize duties, and use computer technology.
• Essential Sensory Requirements: Ability to see and communicate clearly for clinical techniques.
