Job summary

Humana Inc. through CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to support patients in their homes, under the supervision of a Physical Therapist and Clinical Manager.

You will help patients regain strength, mobility, and independence with personalized care aimed at functional improvement. You must have a current state PT license, valid driver’s license, CPR certification, and be able to work with a geriatric population.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric health changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, in-home.
• Perform non-complex manual therapies and modalities as directed by PT.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Report equipment failures or changes to PT or Branch Director.
• Apply infection control and universal precautions in patient care.
