Qualifications

• Graduation from a CAPTE-accredited Physical Therapist Assistant program.
• Successfully passed the national PTA examination.
• Current, active PTA license in Indiana.
• Minimum two years PTA experience; home health preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide physical therapy interventions under supervision of a Registered PT.
• Deliver treatments to relieve pain, restore function, and maintain optimal performance.
• Monitor patient progress toward goals and ensure compliance with physician orders.
• Observe, document, and report patient responses to treatment to the PT/DOR/DON.
• Collaborate with PT on reassessment and plan of care updates.
• Educate patients and caregivers on exercises, safety precautions, mobility techniques, and discharge planning.
• Maintain accurate, timely documentation per agency policies and regulatory requirements.
• Participate in care conferences and interdisciplinary meetings for coordinated care.
