Job summary

Addus Family Company is seeking a dedicated Physical Therapist Assistant to support home and community-based care. You will implement treatment plans under the supervision of a licensed Physical Therapist, focusing on mobility, strength, pain management, and safety for seniors and individuals with disabilities.

You will provide in-home therapeutic exercises, document progress, educate patients and caregivers, and collaborate with an interdisciplinary team.

Qualifications

• Ability to implement treatment plans under supervision of a licensed Physical Therapist.
• Experience with home health or community-based care is preferred.
