Job summary

Human Touch Home Care in Colorado Springs is seeking a dedicated Physical Therapist Assistant to join our in-home care team. The PTA will deliver physician-directed therapy, educate patients and families, and coordinate care with the supervising PT and the broader team in a home-based setting.

Qualifications

• Completion of an accredited PTA program or equivalent 2-year college program approved by APTA.
• Current state license or eligibility to obtain license to practice PTA.
• Two years of experience preferred.
• Acceptance of agency philosophy and goals.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Performs PTA-supervised services planned by the Physical Therapist.
• Assists in preparing clinical and progress notes.
• Educates patient and family during care.
• Maintains documentation and updated care plans per policy.
• Reports changes in patient condition to PT and team.
• Instructs patients/family in home exercise programs.
