Job summary

Human Touch Home Care in Colorado Springs is seeking a dedicated Physical Therapist Assistant to join our team. You will deliver patient-centered care, support a collaborative team, and advance your professional skills in a growing home healthcare setting.

The role emphasizes clinical responsibility, documentation, education of patients and families, and adherence to agency policies—while maintaining compassionate care in home visits.

Qualifications

• Completion of a physical therapy assistant program accredited by CAPTE or equivalent.
• Licensed or eligible to practice in the state(s)
• Two years' experience preferred
• Ability to exercise initiative and independent judgment

Responsibilities

• Performs services planned, delegated and supervised by the Physical Therapist.
• Assists in preparing clinical and progress notes.
• Educates the patient and family.
• Participates in in-service programs and presents as assigned.
• Provides PT prescribed therapy under a written plan of care.
• Coordinates therapy with the PT and other health care personnel.
• Instructs patients and family in home programs and activities of daily living.
