Responsibilities

• Implement in-home therapy services under supervision of a licensed Physical Therapist.
• Follow individualized treatment plans based on the PT’s initial evaluation.
• Educate patients and caregivers on specific exercises and safety techniques.
• Collaborate and communicate patient progress with the interdisciplinary care team.
• Document using AI-assisted tools to ensure accurate and timely charting.
