Job summary

Powerback Rehabilitation To You in Laconia, NH is seeking a Physical Therapist to deliver outpatient rehabilitation services in home and facility settings for older adults. This role offers a $5,000 bonus and flexible scheduling to support patient-centered care.

Must hold a valid Physical Therapy license in the state or have license eligibility. Degree requirements include a Master’s in PT (or equivalent) with related experience.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Must have a Master’s degree in Physical Therapy; or Master’s degree in a Physical Therapy related field in addition to a Bachelor’s degree in Physical Therapy; or Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• Must have a valid driver’s license or reliable, independent transportation to and from the patients’ residence.
• The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.

Responsibilities

• Provide outpatient rehabilitation services in an in-home setting for older adults.
• Evaluate, develop, and deliver physical therapy programs to restore and enhance performance.
• Identify the need for and make referrals to other disciplines as needed.
• Direct activities to promote safety and independence in home and community settings.
• Supervise PT Assistants, temporary licensees, students and support personnel when required.
• Maintain timely clinical and billing documentation and participate in related meetings.
