Job summary

Optum's Assured Home Health is seeking a Physical Therapist to deliver one-on-one in-home care across the Portland Metro area. In this full-time role, you’ll assess mobility, develop individualized care plans and help patients regain independence.

You will work with a multidisciplinary team, maintain accurate documentation, and enjoy a stable schedule (Mon-Fri) with competitive pay and benefits. Sign-on bonus of $10,000 and PTO are included.

Qualifications

• Active Physical Therapy license in practice state.
• CPR certification current.
• Valid driver's license and vehicle insurance.

Responsibilities

• Provide one-on-one in-home physical therapy services.
• Develop individualized plans of care and document patient progress.
• Collaborate with the care team to optimize patient outcomes.
