Job summary

Optum (Assured Home Health) is seeking a Physical Therapist to deliver one-on-one care to patients in their homes in the Portland Metro area. This full-time role offers a $125,000 to $145,000 salary based on a 40-hour work week and a $10,000 sign-on bonus.

You will evaluate patients in their homes, develop individualized plans of care, and work with an interdisciplinary clinical team to improve mobility and independence. PTO, holidays, and comprehensive benefits are included.

Qualifications

• Current Physical Therapy license in the state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance with dependable transportation.

Responsibilities

• Evaluate patients in their homes, develop individualized plans of care and provide treatment focused on improving mobility, function and independence.
• Collaborate with patients, families, physicians and the broader care team while managing visits and documenting patient progress.
