Job summary

Optum is hiring a Home Health Physical Therapist to evaluate and treat patients in their homes, developing plans of care and collaborating with the health care team. You will assess functional status, mobility, endurance, and pain to improve patient well-being and progress toward goals.

The role offers a two-part compensation structure with per-visit and hourly pay, a $10,000 sign-on bonus for external candidates, and comprehensive benefits. Full-time schedule, travel required.

Qualifications

• Current Physical Therapy licensure in state of practice
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Current CPR certification or ability to complete within 90 days of hire

Responsibilities

• Evaluates the patient's functional status and PT needs; assists physician in developing the therapy plan of care
• Provides services within the scope of practice as defined by state laws and the plan of care
• Observes and reports patient responses to treatment to supervising nurse or physician
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning
• Instructs patient and family/caregivers regarding the plan of care and progress toward goals
• Visits patients per Plan of Treatment; completes progress notes per policy
• Completes PT evaluations and develops the PT plan of care within state guidelines
• Reports outcomes of evaluation, goals, and anticipated frequency of care
• Function in any home situation regardless of patient background
