Job summary

Plan A Health is seeking Personal Care Aides to provide in-home care services aligned with individualized care plans. You will help clients with daily activities while ensuring safety and quality standards are met.

Responsibilities include following care plans, assisting with bathing, grooming, mobility, and feeding, using mobility aids, and maintaining infection control and a clean environment. You will also handle light tasks and report any changes to supervisors.

Qualifications

• Minimum 6 months caregiving experience validated through references.
• High school diploma preferred; ability to read/write required.
• Must be 18 years or older.

Responsibilities

• Follow client care plans and complete daily tasks via EVV or electronic timesheets.
• Assist clients with personal care such as bathing, grooming, ambulation, feeding, oral hygiene, and using mobility aids.
• Encourage client independence and self-help activities.
• Maintain safety protocols, infection control, and a clean environment.
• Perform secondary tasks like laundry, cleaning, grocery shopping, and meal preparation as needed.
• Respect client confidentiality, privacy, and rights, ensuring quality care within scope of credentials.
• Assist with medication self-administration and report any safety concerns or changes in condition.
• Notify supervisors of any abuse, neglect, or unsafe environments.
