Job summary

Supportive Connections LLC is seeking a dedicated Personal Assistant Direct Support Staff member to provide one-on-one, individualized support to individuals with developmental disabilities in their homes and communities in California. The role emphasizes safety, dignity, independence, and person-centered planning.

You will help with ADLs, transportation, appointments, and social activities while maintaining accurate documentation and adhering to IPP guidelines and HIPAA policies.

Qualifications

• Must be at least 18 years old.
• Background checks and DOJ/FBI clearances required.
• CPR and First Aid certification preferred.
• Must be legally authorized to work in the United States.
• Must communicate effectively in English, both verbally and in writing.

Responsibilities

• Provide one-on-one personal assistance per client service plan.
• Assist with activities of daily living and personal care needs.
• Support household tasks and maintain a safe living environment.
• Assist with transportation, appointments, and community participation.
• Provide supervision, prompting, and hands-on assistance as required.
• Promote independence, dignity, and inclusion while respecting client rights.
• Document daily services accurately and maintain EVV records where applicable.
• Communicate professionally with clients, families, and supervision.
